Hospital Nacional de Puerto Barrios (the Hospital de la Amistad Japon-Guatemala in the Ministry of Health registry, Santo Tomas de Castilla) is the public hospital; the UK government (FCDO) list of medical facilities marks it as a public facility where you will not need to pay for treatment.
